BloodWorld Consent Form

By proceeding and using the BloodWorld platform, you acknowledge that you have read, understood, and agree to the following:

 

1. Data Collection and Use:

  • I understand that BloodWorld, a blood information management system developed by Casantey Business Solutions Group Limited, will collect and process my personal information for the purpose of providing and enhancing blood donation and management services.

 

  • I consent to BloodWorld collecting and processing the minimal amount of personal information necessary for these purposes, as outlined in the BloodWorld Privacy Policy.

  • I understand that the purposes for processing my information include facilitating blood donation, managing blood and its components (from donation and storage to potential transplantation), and supporting research into new treatments and processes.

2. Data Protection and Rights:

  • I acknowledge that BloodWorld adheres to high standards in handling personal information, consistent with the Data Protection Act 2018, the Data Protection Act, 2012 (Act 843) of Ghana, and the UK General Data Protection Regulation (GDPR) as adapted for BloodWorld's operations in Africa.

  • I understand my rights regarding my personal data, including the right to access a copy of the information BloodWorld holds about me, the right to request amendments to my record if it is incorrect, and the right to be forgotten under certain circumstances (as detailed in the Terms and Conditions).

 

  • I am aware that I can exercise these rights by making a Subject Access Request (SAR) and that BloodWorld has one calendar month (which may be extended in complex cases) to respond to my request.

3. Contact and Communication:

  • If I join the Blood Donor Register or Plasma Donor Register, I agree to be contacted by The National Blood Service (NBS) or its agents via letter, phone, text message, or email with information relating to donation opportunities and follow-up communications regarding appointments.
     

  • I understand that I can change my contact preferences by using the contact form available on the BloodWorld website.

4. Data Transfers and Storage:

  • I understand that my data will be primarily stored in Ghana but may be stored internationally in data centers such as AWS and Oracle data cloud as needed, with appropriate safeguards in place to protect my data in accordance with applicable data protection laws, including the use of standard contractual clauses.

5. Updates to the Privacy Policy:

  • I acknowledge that the BloodWorld Privacy Policy may be updated from time to time to reflect changes in practices or legal requirements. I understand that significant changes will be communicated by posting the updated policy on the BloodWorld website or through other appropriate channels and I am encouraged to review the policy periodically.

6. Contact Information and Data Protection Officer:

  • I understand that BloodWorld is the Data Controller for my personal data and that the Data Protection Officer can be contacted in writing at Casantey Business Solutions Group Limited, 5 Nii Adjei Okpong II Link, Adjiriganor, Accra, or by email at privacy@casantey.com.

 

  • I am also aware of my right to contact the independent data protection authority in Ghana for independent advice on data protection, privacy, and data sharing issues, with contact details provided in the Privacy Policy.
